Please call the office to schedule. Full payment for the Medical Exam is due upfront at first visit (we do not submit to insurance). Must pay with credit\/debit card, money order, or certified check (no cash or checks).<\/p>\n<p><strong>You need to bring:<\/strong><\/p>\n<ul>\n<li>Valid, government issued ID or Passport*<\/li>\n<li>Any and all vaccine records<\/li>\n<li>Payment for Immigration Medical Exam (includes both visits) &#8211; $1000<\/li>\n<\/ul>\n<p><em>*Alternative proof of identity may be accepted for children under 14 years of age that shows name, date and place of birth, parents\u2019 full names and other identifying information.<\/em><strong>\u00a0<\/strong><\/p>\n<p><strong>First Visit includes:<\/strong><\/p>\n<ul>\n<li>Quantiferon (blood draw) &#8211; patients 2 years &amp; older<\/li>\n<li>Varicella Titer (blood draw) &#8211; patients 2 years &amp; older<\/li>\n<li>Syphilis (blood draw) \u2013 patients age 18-44<\/li>\n<li>Gonorrhea (urine test) \u2013 patients age 18-24<\/li>\n<\/ul>\n<p><strong>Available Vaccinations:<\/strong><\/p>\n<ul>\n<li>Tdap<\/li>\n<li>Hepatitis B<\/li>\n<li>Influenza (seasonal September &#8211; July)<\/li>\n<\/ul>\n<p>After First Visit \u2013 At check out you will be given a list of all vaccine records that you still need to provide to schedule the Medical Exam.\u00a0 You can get these done at the Health Department or your family physician.\u00a0 If a chest x-ray is determined necessary based on your Quantiferon results, we can complete the x-ray at our office for an additional $60.\u00a0 Once we have received all records and test results, we will call you to schedule the Medical Exam with our Civil Surgeon.<br \/>\n**<em>To ensure the exam can be completed, the final visit must be within 3 months of the first visit.<\/em><\/p>\n<h2><u>Final Visit<\/u> \u2013 Appointment Required<\/h2>\n<p><strong>**If an interpreter is needed, you must bring one with you to exam**<\/strong><\/p>\n<p>You must bring your valid, government issued ID or Passport again.\u00a0 Our civil surgeon will perform your immigration medical exam at one of our 5 locations.\u00a0 The I-693 form will be completed and sealed for you to submit to USCIS.<\/p>\n<h2><u>Frequently Asked Questions (FAQs)<\/u><\/h2>\n<ol>\n<li><strong>Are there other costs above the $1000?<\/strong><br \/>\nIf a chest x-ray is determined necessary based on your Quantiferon results, the additional charge is $60. If you elect to receive any vaccinations at one of our facilities, there will be additional cost for those vaccines.<\/li>\n<li><strong>What vaccination records do I need before scheduling the Medical Exam?<br \/>\n<\/strong>Tdap or DTaP, Varicella, Hepatitis B, Rotavirus (under 8 mths), Hepatitis A (12 mo &#8211; 18 yrs), Influenza, OPV\/IPV (2 mo and older), MMR (Measles, mumps, and rubella), HIB (under 5 yrs), Meningococcal (11-18 yrs), Pneumococcal (2 &#8211; 59 mths and over 50 yrs).<\/li>\n<li><strong>If I don\u2019t have records of my vaccines, what do I do?<\/strong><br \/>\nYou will have to get the vaccines again. These can be completed at the Health Department or some local pharmacies.<\/li>\n<li><strong>How long does the process typically take?<\/strong><br \/>\nTotal process typically takes 3-4 weeks. This can vary depending on how quickly we receive your Vaccine records.<\/li>\n<li><strong>What is included in the Medical Exam?<\/strong><br \/>\nOur Civil Surgeon will review all vaccination records and complete a basic physical exam with a larger focus on medical history.<\/li>\n<li><strong>What happens if the final visit is not scheduled within 3 months of my first visit?<br \/>\n<\/strong>Depending on the circumstances, you may still be able to have the exam completed but you should contact the clinic for further instructions. Based on the situation, they will tell you whether the exam can still be completed or whether you will have to start the process all over again, including paying the $1000.<\/li>\n<li><strong>When I went to my appointment with immigration, I was told that there was an issue with my paperwork (not expired paperwork). Can you help me?<br \/>\n<\/strong>\u00a0 Please contact our office and ask for a coordinator to assist.<\/li>\n<\/ol>\n<ol start=\"8\">\n<li><strong>If I show up late for my final visit, will I still be seen?<br \/>\n<\/strong>It is best if you show up early. If you are late, we will try to get you in, but it will depend on the civil surgeon&#8217;s schedule.\u00a0 If you arrive more than 15-20 minutes late, you may have to reschedule.<\/li>\n<\/ol>\n<p><strong><u>Health Departments<\/u><\/strong><br \/>\nSt. Joe County: 227 W.
