PAWS Program

Our mission is to improve the health of youth, and to increase their capacity to learn, by bridging the gap between education and health, through personal responsibility and  the support of families, communities, and schools working together.

Hours of Operation:  Monday through Friday, 8:00 am to 5:00 pm, closed 12:00 pm to 1:00 pm.

Our Services

The School-linked PAWS/CAHC program promotes the health of  adolescents and their families by providing important primary, preventative, and early intervention health care services.These services are aimed at achieving the best possible physical, intellectual, and emotional status of adolescents by providing  services that are high quality, accessible and acceptable to youth.

A variety of Health Services are available, including:

  • Primary Health Care
  • Preventative Care
  • Comprehensive Health Assessments
  • Sport Physicals (MHSAA approved)
  • Camp Physicals
  • Well Exams
  • Referral for Specialty Care
  • Treatment of Acute Illness/injury
  • Basic Laboratory Services and tests
  • Chronic Disease Management
  • Medication
  • Immunization Assessment/Administration
  • Health Education/Risk Reduction Programs
  • Mental Health/Counseling Services with a Licensed Masters Social Worker
  • Nutrition Education/Counseling
  • Referral for Oral Health Services
  • Medicaid Enrollment Assistance
  • Confidential Services—include the following; testing and treatment of STI and HIV, birth control, and pregnancy testing. Minors above the age of 14 can also receive counseling services not to exceed 12 visits over 4 months.
  • PAWS/CAHC is just like any other medical office except the focus is placed on the specific needs of youth.
  • Anyone between the ages of 10-21 can receive services, regardless of their ability to pay.
  • All insurance plans are accepted, including Medicaid and Medicaid Health Plans.  Insurance will be directly billed, co-pays may be collected at time of service.
  • A signed parental consent form is required on all clients ages 10-17 for all services with the exception of confidential services.
  • Three Rivers Community School students may receive services during the school day (High school students may walk over, while Middle School and elementary students are transported).
  • Appointments are not necessary but are strongly recommended.
  • PAWS/CAHC will communicate with the adolescent’s  primary care providers to manage ones health care.
  • PAWS/CAHC is partially funded through a grant received by the Michigan Department of Community Health and Michigan Department of Education.
