Top 5 Things to Know about Benefits

1. Lose Your Insurance Card?

If you mistakenly tossed the envelope containing your medical insurance card in the trash, don’t worry. If you are new to the Beacon health plan, please call Meritain at  800.925.2272 and they can help you and your family members get new cards. If you are an existing member of Beacon’s health plan, you can get new cards by visiting mymeritain.com.

2. Medical Claim? Do This First

Before any medical claims can be paid this year, you have to complete Meritain Health’s Other Insurance Coverage (OIC) Form. This form can be found on Human Resources website, under the Forms tab or from the 2014 Benefits Enrollment Guide.

3. Start Saving Now

Just a reminder, if you enrolled in the Health Savings Account for 2014 and have already received your HSA Card, your card will need to be activated before it can be used. Visit the Chase HSA accountholder website at chasehsa.com or call  1.866.295.9173. You will need your date of birth, and last four digits of your Social Security Number.

Did you know, you can view and manage of your HSA account online?! Try it out – register your HSA card online at chasehsa.com. If you have any questions about your HSA, please call the HSA Customer Service at  1.866.566.7101.

4. Flex Those Funds

Do you have unused Flexible Spending Funds in your 2013 Flex Account still available for reimbursement? It’s not too late to be reimbursed! Any unused funds can be refunded by completing a Flexible Spending Reimbursement Form. However, you can no longer use your Benny Card for 2013 expenses. Reimbursement forms can be found on the Human Resources website, under the Forms tab. You have until March 15 to submit reimbursement forms.

5. Pre-certify and Save Money

To help control expenses, under all Beacon medical plans there is a listing of medical procedures (outpatient surgery) and services (physical therapy) which must be pre-certified. For example, if you have a planned knee surgery, you must pre-certify two weeks in advance of the procedure. By not pre-certifying the medical service procedure, your insurance coverage drops by a whopping 50 percent. A complete listing of services which require pre-certification can be found in the 2014 Benefits Enrollment Guide. Pre-certifying instructions can be found on the back of your Meritain Health insurance card.
