Requesting Your Medical Records
Note: Beacon Medical Records offices are now open for in-person medical records requests beginning Monday, February 1. The offices are observing enhanced safety measures established at our hospitals including, but not limited to, symptom and temperature checks, masks for all individuals, and social distancing.
There are a few ways you can request copies of your medical records, depending on the type of information you need. You can access portions of your electronic medical record online through the Patient Portal or through the Medical Records office.
For the most convenient access to your records, sign up for MyBeacon. MyBeacon puts your Beacon Health System services and medical records at your fingertips. Save time and stay up to date on your health care.
How to Request Your Medical Record
Complete the correct form and mail, fax or drop off at the corresponding Beacon site listed below. You may also email forms to gro.metsyshtlaehnocaeb@noitamrofnifoesaeler. Please include a copy of your photo identification along with your request or submit the request in person.
