Paragon, of St. Joseph County in South Bend, Indiana, is actively seeking a full-time Primary Care Physician to join our dynamic and collaborative team.
In this role, you will have the opportunity to provide meaningful care for roughly 1,600 employees throughout the county. This is a client-based model where you will get to spend time with your patients, building a relationship, and creating custom wellness plans for their needs. Our work-site/near-site clinic model gives physicians the opportunity to practice medicine the way it was meant to be—focused, relationship-driven care, without the volume pressures of traditional clinics. You’ll work with a stable employee population, have the time to build meaningful patient relationships, and focus on prevention, wellness, and early intervention. Our primary care services are designed to meet the comprehensive needs of our employee population through a patient-centered, proactive approach to care. Providers deliver a full range of services, including evaluation and management of acute illnesses, annuals and sports physicals, chronic disease management, preventive care, and in-office procedures such as biopsies and joint injections.
In addition, patients benefit from robust, individualized wellness programs tailored to support health goals such as weight management, smoking cessation, and diabetes management.
The clinic offers convenient on-site diagnostics, including both POC and LabCorp laboratory services, as well as an on-site pharmacy with an established formulary that includes select vaccines. POC testing capabilities include glucose, HCG, strep, COVID-19, influenza, urine albumin/creatinine ratio, and urinalysis with microscopic examination. On-site EKG testing is also available, allowing for efficient, comprehensive care delivery in a single location.
Qualifications:
- 5 years of experience as a Primary Care Physician is preferred, but we are also interested in speaking to new graduates
- Must be comfortable supervising APCs
- Comfortable caring for pediatrics and geriatrics. Will oversee patients typically around 5 years old and up, including retirees
- Previous experience working in ER or Urgent care is strongly preferred

Practice Highlights
Beacon Health System Worksite Clinics remove existing barriers and make receiving care simple and easy for patients when they need it. This means that minor ailments do not escalate into major health events.
All services at our Beacon Health System Worksite Clinics are provided at no cost to the patient. Exams, procedures, consults, case management, chronic disease management and wellness coaching, among others, are all performed by our medical team without any financial responsibility to the employee/patient. No co-pays, no deductible contribution, no filing claims, no reimbursement. The fee structure centers around a PEPM model.
Patient appointments are scheduled in 20–40 minute time blocks, allowing for thoughtful, high-quality care. Protected time is built into the schedule for charting and patient messaging, supporting work–life balance and reducing after-hours administrative burden.
This is a well-established, mission-driven primary care practice focused on delivering comprehensive, high-quality care at no cost to employee-patients. Providers enjoy the feel of a private practice environment, which allows for greater personalization, continuity of care, and meaningful long-term relationships with patients.
The practice has been serving the community for nearly 15 years and continues to grow. In addition to the main location, the organization has expanded to include three satellite clinics operating under similar APC-supported care models, with clear indicators of continued growth and sustainability.
