What to Bring to Your First Appointment
Your insurance card and driver’s license
Physician referral forms (if required by insurance)
Prescriptions or over-the-counter medications you currently take
Pertinent information about your medical and surgical history
Any recent X-rays or relevant medical records you may have
What is HIPAA?
HIPAA (Health Information Privacy and Accountability Act) is a federally mandated patient privacy law that states we are not allowed to discuss any of your medical care with anyone but you. If you would like us to be able to speak to friends or family members about your health care, you will need to fill out a special form — which may either be included in your patient packet or available upon request — indicating your permission to do so. You may indicate on the form that you want us to discuss only certain parts of your care with your friends or family members. By law, this form must be filled out each year.
We try our best to stay on schedule; however, emergencies sometimes arise. If we are seriously delayed, we will try to notify you prior to your appointment. If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This will allow us to better serve all our patients.
We feel it is important for patients and their families to understand as much as possible about their treatment. We have a variety of health-related pamphlets for you to take home.
Fees are determined by the time spent and vary with the severity and complexity of your medical problem. Please feel free to ask any questions you might have concerning fees.
Payment/co-payments are due at the time of your office visit. You may pay by cash, check or credit card and, if necessary, we can arrange a payment plan. For questions regarding your bill, please call:

If you have insurance we will file your claim for you. Please be sure to have your current insurance card available. If we are a participating provider for your insurance, you will be responsible for your co-pay, deductible or percentage, at the time of your office visit. If we are not a participating provider, or you do not have insurance, payment is expected in full at the time of service.
