Outpatient palliative care clinic

For patients who are not hospitalized, outpatient palliative care services are available by referral. The clinic helps patients manage symptoms, navigate treatment decisions and receive ongoing support while living with serious or chronic illnesses. A referral is required for outpatient palliative care services
